DOCUMENTS REQUIRED FOR ADMISSION

  1. Completed application form obtained from Selection Committee in Malmö.

  2. Photocopy of the “Upper Secondary Education Diploma” in native language, certified as stated in the booklet.

  3. English or Polish translation of the above Diploma by a government authorized translator or by your school. Both documents (2 and 3) must be submitted.

  4. Passport photocopy - the page with photo and personal data.

  5. Birth certificate in English (Fødselsattest from Folkeregisteret, personbevis).

  6. Four recent passport size photographs (3,5 x 4,5 cm),
    print your name on the back of each photo.

  7. “Statement of Eligibility for Higher Education” form,
    VHS office – ONLY Swedish students.

All documents must always be sent to dr Barbara Jedlińska,
Truedsgatan 2-F, 215 67 Malmö, Sweden.

Important Note
In order to apply to the Selection Committee you do not need to send us the whole set of documents at once. To obtain some documents like an "apostille" or translations you need time.

  • You can submit them in your next letter to Malmö.
  • You can apply for admission even if you are still attending the last year of your high school. In this case you need to send us your marks from the first and second year of your high school (samlat betygsdokument). You do not need to certify this document. Certification is only necessary on final school diploma.

FÖRKLARING TILL TABELLEN


Att: 2. Från 2005 gäller följande EU bestämmelser rörande offentliga handlingar, bl.a. slutbetygen: en kopia av svenska/norska slutbetygen måste

  • intygas av skolans administrativa personal med namnteckning och namnförtydligande, kopian måste också
  • stämplas med skolans stämpel eller sigill,
  • kopian måste sedan skickas vidare till Notarius Publicus som bestyrker den med s.k. ”apostille”.

Notarius Publicus hittar du i gula sidorna eller på www.eniro.se. Vi kan rekommendera Notarius som är redan bekant med den typen av ärenden och som dessutom har förmånliga priser: Heinz Clauss, Nygatan 56, Box 43, 231 21 Trelleborg. Tel. 0410-430 50.

Att: 3. Översättning av slutbetygen/vitnemål och kompletterade ämnen:

  • till Gdansk Medical University måste vara på engelska eller polska, utförd av en auktoriserad översättare. Det accepteras översättning från skolan, dock måste den vara stämplad med skolans stempel eller sigill och signerad
  • till Wroclaw Medical University översättning till polska endast accepteras.

Att: 8. Intyg angående grundläggande behörighet till högskoleutbildning från VHS skall vara
