If you decide that you would like to study medicine in Poland, you must send us details of your qualifications from high school BEFORE we can send you any application forms.


Please include the following points in your letter to us:

1. Family name and first name
   
   
2. Date and place of birth
   
   
3. Citizenship (and nationality)
   
   
4. Present address, phone, mobile, fax, Email
   
   
5. Permanent address (e.g. parents address) including phone
   
   
6. High School Certificate - xero-copy of original or if you now a student of high school - copy of your grades up to now
   
   
7. Any complementary subjects or courses
   
   
8. Working experience in health and medicine
   
   
9. Why you are interested in medicine - short statement
   
   
10. Any additional information
    
    

The Selection Committee includes medical doctors who will evaluate your letter and decide on, whether you are a suitable candidate to apply for these studies. Only then, will you receive all the needed information as well as application forms. As we already have a number of candidates, we would advice you to write as soon as possible.
